A lot of people with OCD are tormented by thoughts that they will hurt those they love--physically. It has no connection with reality. Someone with OCD, who is tormented by those thoughts, will certainly never carry through with them. Do you have any accompanying rituals when you have the thoughts?
Often SSRIS in high doses are prescribed for OCD. It sounds like you are on a low dose of Zoloft.

As the other poster said, these are small doses-- even my starting dose of Risperdal was higher. And I took 400mg of Effexor. And that was just for social anxiety at the time.
I can't believe you're not on a benzodiazpiene while waiting at least for these meds to kick in. A simple Klonopin 3x/day is typically extremely helpful for obsessive thoughts... if you feel like a thought is "repeating" in your head, it would basically stop that... assuming you take the right dose for your body.
But you're on low doses, especially for your symptoms. And frankly, if your doc isn't giving a benzo to you for what you just explained, I really would consider talking to him about that. You have a very severe and painful problem you're suffering with, that's affecting your life.
Ask him why I'm given Klonopin daily so I can feel comfortable with picking up girls while you worry whether you won't hurt your son.
I honestly think doctors that don't prescribe a benzo in this case need to go to jail.

I think your distressful feelings could be coming from many different areas. You need to talk to your doctor immediately about your violent thoughts. It could be from a disorder you haven't been diagnosed yet, could be the meds interacting with each other, could also be that you are having a different reaction to a med. Someone mentioned to you that Klonipin would help, I think that may be an idea and you should mention it to your doctor. If you are seeing a family doctor, most of them aren't prepared to deal with psychotropic meds or mental illnesses. If you are seeing a psychiatrist or nurse med, you need to be an advocate for yourself and let people know what's going on with you. You were smart to have your son stay with family. What I would want for you is to be feeling level. Good luck.

i am also a diabetic. i take metformin. i wonder why he is not prescribing klonipin. i will ask him the next time i see him. this dr is in scott and white so i thought he was giving me the best meds. he wanted to give me prozac but i have heard bad things about. i was on just 50mg of zoloft for a year and half but when he upped it to 100mg i had the thoughts worse. now im on risperdal and effexor and the thoughts are hardly there. but i will ask him about klonipin
