Thanks, yxibow. Anyone know whether a patch medication requires a larger dose on someone with a thicker than average layer of subcutaneous tissue (i.e. fat)? Cecilia

I doubt adiposity has that much effect, but I am only guessing. Frankly, it's ultimately moot because the dose you respond to is the dose that works.
Also, BMS has the emsam monograph on their web site.

The other thing that worries me about Emsam is that the most important thing I'm learned about meds over the years (from the school of hard knocks) is start at the lowest possible dose and go up s-l-o-w-l-y. Unfortunately, that won't be possible with a patch, 20 mg is the lowest dose. Anyone know what the half life of this drug is? Cecilia

I have the PDR insert for Em-Sam it can be found on Bristal Meyer Squibbs site as they are the actual manufactuer. I have been tracking this for some time now (over a year). Low dose selegiline works for me but most Psych doctors won't prescribe it here as it was not on label for that indication. So I've been waiting for Em-Sam to be approved which happened on Feb 28, 2006.
The only side effects I got from low dose Selegiline was insomnia which subsided after a couple of weeks & sporadic short duration tremors (about once a day lasting maybe 2 minutes).
What I got in return was relief from depression & much more energy.
I only hope that Em-Sam doesn't put me over the top or act significantly different tha the oral version of Selegiline

Was taking oral selegiline at 2.5 mg a day. that resuls in about 1/40th the level of selegiline delivered by the patch a day but only slightly less metabolites than the patch (about 75% compared to the patch).
That is why the concern of the pach over doing it.

I haven't got much response from oral selegiline at 10mg a day. I'm hoping the patch will be better. I think I need both the MAO-A & B inhibition.

Cecelia-
My girlfriend is transitioning to selegiline from another MAOI. Basically, the plan is to get her on EmSam. We think that she can come up on selegiline slowly using the oral form and the swap over to the patch once it becomes available. Seems most doctors don't know much or are afraid of MAOIs, so not much direction on this matter seems available for us.
Anyone out there know if this is a viable solution to titrating up to the doses provided by the patch?
Also, selegiline seems pretty easy to titrate up on. It's going up relatively fast and smooth. Hope the same pans out for you.
