Like if you take your meds & vitamins at once may some of them not metabolised, digested?

Certain medications are directly affected by vitamins, but they are few in number. The main one that comes to mind is warfarin, which is not a psychoactive substance.
Warfarin, usually taken to prevent strokes, is affected by foods and vitamins which contain significant amounts of Vitamin K and Vitamin E, as they may dangerously increase or decrease warfarin levels and the ability to stop clotting or inversely increase the potential of bleeding.
Ask your doctor if you're not sure about what vitamins and minerals and other herbal products you may be taking.
