Any substitute for long term Nardil Use?

Posted by Jedi on April 23, 2008, at 3:05:54

Hi guys,
Has anyone come off of Nardil and found a cocktail of antidepressants and augmentors that worked as well for atypical depression and social anxiety? I can't take Parnate because of a rare spontaneous hypertensive episode. I have never tried Marplan.

Nardil is the only thing that has worked for me. I augment with clonazepam for the social anxiety and use 25mg of Seroquel to handle the med induced insomnia. I've been on Nardil for most of eleven years with some time off for other med trials. Every time I have come off Nardil my major depression has returned.

I'm prepared to stay on high dosage Nardil the rest of my life, if it is the only thing that works for my major depression. The weight gain plus delayed ejaculation are my worst side effects. Nardil causes extreme carbohydrate cravings for me plus I believe changes my metabolism to a lower level. At one time I had gained more than 100 pounds on Nardil. Two hours of intense exercise daily, and I can lose weight, even on high dose Nardil. It is so easy to let the weight come back on, though. I have augmented with bupropion, though contraindicated, which helped the weight somewhat.

I would like to hear from any ex-Nardil or ex-MAOI users that have found another antidepressant that has worked for atypical depression. I am treatment resistant with over 45 different combinations that have not worked, including most all of the SSRIs. Any help would be appreciated.
Be Well,
Jedi

Jedi are you hoping to be able to change meds? I'm glad you are better but understand the concern for weight gain. Good luck hope some answers for you. Love Phillipa

hi Jedi,

First you're a great supporter here...I can't give you a personal advice, well, there is marplan and there are maybe new MAO's, partial GABA-A receptor agonists and so on coming in some years:

For weight loss, you can try metformin, and as potentially more effective alternatives to bupropion, methylphenidate and modafinil. Zonisamide can also be effective for weight loss - I just read about it a few minutes ago.

As for replacing Nardil, the best bet would seem to be Marplan, as it's the most similar. Other MAOI options include selegiline (oral or transdermal) and rasagiline.

If you haven't tried memantine, that may be an interesting option.

I suspect you've tried just about everything else.

Jedi, years ago after Marplan was pulled from the market, I tried a variety of meds and finally had some success on Celexa/Wellbutrin/Adderall combination. That was for a few years. I really think there was something to the Adderall that made the combo effective. Marplan was great the forst time I took it but did not work the second time.

Now I'm back on Nardil 60 mg (second time, less effective than first time, original formula) but recently finding that a smidgen of an amphetamine boosts the effects of Nardil alone, but never more than 2.5 to 5.0 mg as beyond that, you may create the hypertensive crisis.

Worth a try perhaps?

Hi Cee,
My normal dosage of Nardil for years was 90mg per day. I have been up to 105 and 120 but only for a short time. My wait gain and delayed orgasm side effects are much worse at the high dosages. Right now, I'm trying to cut back to 60mg in hopes of reducing the carb cravings. I went to 45mg for a while but the MAO Inhibition at this dosage was not enough to hold back my major depression. So far so good at 60mg, but have only been there for about a week.
Take care,
Jedi

I took 60 mg per day (I'm a big boy, about 250 lbs at the time). It was quite effective and I couldn't have handled any worse side effects.
