How do I apply for disability? Medical insurance?

Need Advice:

I have decided to give in to my illness, bipolar disorder, and accept my disability. I don't have any idea of how to proceed!

At my place of employment I have short term disability (which I am currently on), and long term disability available.

My long term disability will only last 2 years because I don't have a "real" illness such as heart disease which I might have caused by my lifestyle(this is a dig at the way insurance is for bipolar people in this country).

What is available from SSA and how do I go about getting it!

In addition, what do people do about health insurance after being dropped by the employer's policy? I live in Tennessee, but TennCare seems iffy! Anyone have the answer?
