Ali - There are several drug interactions happening, but it would be hard to pinpoint which ones are causing your problems. All four drugs, lithium, carbamazepine, lorazepam, and Zoloft, can all cause cognitive problems by themselves. This "can" be exacerbated by taking them together.
Taking Zoloft and lithium together "can" cause overstimulation of the 5-HT1A receptor leading to serotonin syndrome, which has some cognitive manifestation. This combination "can" lead to lithium toxicity, which also is related to cognitive difficulties.
Zoloft and carbamazepine, alone or together can interaction with a couple of components in grapefruit juice, raising the levels of the drugs. possibly causing cognitive dysfunction. Also, the two together "can" cause CNS depression, thus problems in thinking. A similar situation "can" happen when mixing carbamazepine with lorazepam.
Carbamazepine and lithium taken together "can" cause confusion.
So, take your pick. These interactions do not necessarily happen, but "can". Take these with a grain of salt. These combinations are used quite often, with little problem. If you are really concerned, ask your doc about a wholesale med change, especially if this one is interferring with your quality of life. Your doc may see the above as an exaggeration, which it is, to some extent, but the literature is out there.
