I have been reading your posts and I was wondering if you have considered a trial of modafinil. Modafinil is available in Canada for the treatment of narcolepsy under the brand name of Alertec. Here are two URLs with information about this med:

In the U.S., modafinil is a Schedule IV drug and is much easier to prescribe than Schedule II psychostimulants such as Ritalin and Adderall. Your doctor may be willing to give you a trial of modafinil whereas s/he might not even consider a trial of one of the psychostimulants.
Modafinil has also been used successfully (off-label) to reduce the fatigue associated with conditions such as multiple sclerosis, fibromyalgia, chronic fatigue syndrome, etc.
There is a study (in Medline) that suggests that modafinil may be effective in the treatment of adult ADHD:
