You might still profit from TCA monotherapy.
Nortriptyline gives you much of the benefit of amitriptyline, but without the somnolence. I experienced sleepiness during the first week with nortriptyline, but thereafter, I hardly knew that I was taking the drug. I am still taking it. I feel that it is a cleaner drug than the SSRIs in terms of sexual and motivational side effects. Also, it produces less brain fog.
I don't see a problem with combining tricyclic antidepressants. I think it would depend on which two you want to combine and at what dosages, and what your reasons are for combining them.
Small amounts of doxepin (Sinequan) or trimipramine (Surmontil) have been added to other TCAs in the past to act as a hypnotic and anxiolytic. Trimipramine in particular is good for maintaining improved sleep architecture.
