That's an interesting question. I wonder too.
I wonder if there is still anorexia but it comes with a different rationale in traditional Muslim cultures without exposure to Western media....like the Victorian girls who were pursuing an invalid model rather than "sex appeal" or the medieval saints who were anorexic for ostensibly religous reasons struggling against the desires of the flesh rather than trying to reshape it.
I know in one eating disorder program the patients and staff were not allowed to wear revealing clothes (or makeup!) to take the focus off appearance. I don't know how that worked though it sounded like a good idea to me.
