Anger Control/Rage Question

Posted by w.childress on March 6, 2001, at 14:57:16

I'm looking for recommendations about helping an 11-y-o female who is severe ADHD, often Oppositional, and occasionally experiences out of control rages. She takes 40 mg Ritalin (15/15/10) each day for the ADHD. She has been taking 20 mg Prozac for the past two years. This eliminated the Rages for approximately 18 months. When the rages returned, the Prozac was increased to 40 mg. but the Rages continue. After the rage runs its course, she is often remorseful about the things she has said to her parents during the rage. Any suggestions for bringing the rage back under control?

Yikes. sounds nasty. All I can offer you is the experience of an adult with ADD who also has a child with psychological "issues."
Meds: ask for a trial of concerta; your daughter may respond better to the time release formula; many, many adults and children
cannot take ritalin because of anger or problems with crashes; might you consider a switch to adderall? It works for a longer time,
has no discernable crash, and is often better tolerated than ritalin.
what does your pdoc think about the prozac? would he or she be willing to switch if all else fails? is your daughter taking this
for depression or rages??? if for depression, I would consider fooling with this as a last resort. rages might be better handled either
with some therapy or a different drug.
NOW, before you change anything, try to think about what has changed for your daughter; I know the odds are that she's grown, is in
pre-pubescent hell or PMS or whatever that torture was :):) but perhaps this is social? A young woman with ADD/HD has a LOT to be mad
about, especially around puberty.
I hate to say this but if she doesn't see a good therapist already, now is the time to start. ADD/HD is not simply treatable with
meds. believe me I know.
sorry I can't be of more help. Have you tried the ADD site at About.com? They have great info on meds and comorbid stuff like anger etc.
I wish you well, and boy do I feel for you!
S.

I agree with the previous poster's advice. I'd like to add that Risperidal is often used to control rages. Ask your doctor about it.

There is a good book out called THE EXPLOSIVE CHILD and the author's last name is Green, I think.

My son has Asperger's Syndrome, a mild form of Autism, and many of the characteristics overlap those of ADHD. And I know alot of the kids who have rage control problems are given Risperidal. However, it does cause weight gain and appetite increase. She might also benefit from therapy to help her control her anger and begin to recognize her triggers.
