Hmm. That's difficult. I would prefer to do it in person rather than email. Pity your boss is not a man, because they are less likely to ask questions. If she asks what is for you can say it's a regular medical appointment. Could you offer to go in to work early on the Thursday to make up the time? I have never asked my boss. I just leave early on a Tuesday. No-one ever questions it. They probably think its something to do with my childcare arrangements. But I have been in my job for a while, so its a bit different.
